Tax season is coming, where is my 1095 form?
Every year, as soon as the tax filing season is approaching, many customers will ask “My 1095 form is missing, can you send me another form?” It can be seen that many people don’t know exactly how the 1095 form came about.
First of all, it is necessary to understand which 1095 form you should get. There are three types of 1095 forms, including Form 1095-A, Form 1095-B and Form 1095-C.
Form 1095-A
If your personal health insurance or family health insurance is applied through Covered California, you will receive the 1095-A form from Covered California before mid-February, which will have person’s name, social security number, date of birth, address, coverage date range, and most importantly, the amount of premium subsidy received each month will be displayed on the 1095-A form. When filing taxes, accountants usually require taxpayers to provide 1095 -A form to check whether the subsidy is correct.
Form 1095-B
If you apply for personal or family health insurance through an insurance broker directly with the insurance company, such as Blue Shield, Anthem Blue Cross, Health Net or Kaiser Permanente, the insurance company will directly send the 1095-B form to each policyholder, which will also have the name, social security number, birthday and address, but unlike the 1095-A form, there is no subsidy amount, because through the insurance company, there is no subsidy for direct application insurance. The 1095-B form will only tell you which months are insured from January to December. In 2022, if you do not have health insurance for more than 2 months, you will still be fined. If you applied for your health insurance through an employer company, you will also receive Form 1095-B.
Form 1095-C
If the company you work for is a large company with more than 50 full-time employees, in addition to the 1095-B form sent by the insurance company, you will also receive the 1095-C form from the employer. The 1095-C form is mainly for the employer to confirm that the monthly premium paid by each employee is affordable, otherwise the employer will be fined.
After understanding these three 1095 forms, what should I do if I really can’t find the 1095 form when filing taxes? If it is 1095-A, you can log in directly to your California Coverage online account, and there will be a 1095-A file on the portal. Or you can call directly to California Coverage and ask for a new form to be resent. If you have 1095-B, you only need to call the insurance company to request re-delivery. Basically, you have to contact the insurance company. It is useless to call the insurance broker. If you find that the information on the 1095 form is incorrect, be sure to contact Covered California or the insurance company to correct your information, so as not to delay your tax filing time.
